Information/Equipment to Bring To Evaluations
- Proper clothing. Kids attire should be suitiable for a physically demanding activity, suitable for the facility (indoor or outdoor). If the evaluation is outdoor your childs clothing should be suitable for the weather conditions. Please refrain from sending your child in jeans to a soccer evaluation.
- Proper shoes/cleats for the field surface (skateboard shoes/hightop street shoes do not prerform well). If evaluation is in a gym, or on a hard court surface at the soccer centre bring gym shoes not cleats.
- Refreshments: Please make sure your child has water or some sort of beverage as the evaluations are demanding.
- Registration Cheque. If you registered and did not pay by credit card and did not mail in your cheque, bring the cheque and make sure you receive a receipt.
- Volunteer Bond Cheque. You will be required to provide a volunteer bond of $50 per registered child and this cheque will be returned at the end of the season if you have assisted Lakewood in some manner.
Understanding the evaluation schedule
- Example: U12 Boys A-H means that all boys whose last name starts with a letter in the range of A to H, and who registered for U12 boys, regardless of the division they registered for, should show up at that evaluation session.
- Example: References to divsions. If a evaluation time references a division (premier, div 2, division 2) then only those children who have registered for that division should attend the evaluation.

The following form provides parents and players with an understanding of how players are evaluted. The form used by evaluators during tryouts is a simplifed version of following form but kids are assessed based on the skill areas discussed in the player evaluation form. This form can be used by parents, players or coaches to assess a players progress over the year.
